Standalone disclosure
An employer may request a consumer report about you for employment purposes through a qualified third-party consumer reporting agency. The report may include information about criminal history, employment, education, professional licenses, driving history, identity verification, or other information permitted by law. This page is separate from the employment application and does not require you to waive any legal right.
Your authorization
No report may be ordered through SentryHire unless you affirmatively authorize the specific request using your signed-in account and typed name. Authorization is voluntary. A typed name records your electronic authorization but is not notarized or independently identity-verified.
SentryHire's role
SentryHire is not a consumer reporting agency and does not independently search criminal, credit, court, or social-media databases. Screening must be performed by a separately contracted provider that represents it complies with the Fair Credit Reporting Act and applicable state and local law. Until such a provider is configured, SentryHire records disclosures and authorization only and does not order reports.
Employment decisions and disputes
If an employer may take adverse action based on a consumer report, the employer and screening provider are responsible for the required pre-adverse-action notice, copy of the report, Summary of Rights, reasonable time to respond, and final adverse-action notice. You may dispute inaccurate or incomplete information directly with the screening provider and may also flag the request in SentryHire.
Important rights
Review the Federal Trade Commission's employment background-check rights guidance. New York City and other jurisdictions may provide additional protections. This disclosure is an operational template and should be reviewed by qualified employment counsel before live screening is enabled.
